The published schedule reads: 07:30 pickup, 90 minutes on the bus, three hours at Cu Chi, lunch, two hours in the Mekong inclusive of boat transfers, back around 19:00. One first-person account of the same product ran 07:00 to 20:00 — thirteen hours — and described the Mekong half as one boat ride after another until they wished the tour would end.
The 13,742 reviews prove it sells. They do not prove it is the right buy. A product can be the market leader and still be the wrong purchase for the person reading the listing, and that is exactly what the rating gap says.
Who should buy it anyway?
Someone with exactly one free day in the city who consciously prefers a 13-hour highlights blur to seeing one thing properly.
That buyer exists and we are not going to pretend otherwise. If you fly out tomorrow, you will never be back, and ticking both boxes matters more to you than either experience, the combo does what it says. Go in expecting a long bus day.
Everyone else: pick one. A half-day at Cu Chi plus a free afternoon in the city beats the combo, and it costs less.

Cu Chi and the Cao Đài Great Temple at Tây Ninh are both northwest of the city, about an hour apart, on the same road.
This is the geographically coherent full-day pairing, and it is undersold relative to the Mekong combo precisely because it is less famous. You get the noon ceremony at the temple — visually the most extraordinary thing within reach of Saigon — and the tunnels, without crossing the city in between — see the temple on Google Maps for how the two line up.
